Insurance:
- It is your responsibility to check with your insurance company to ensure coverage prior to your appointment.
- Please note that some plans have a waiting period before coverage begins on certain services. Some plans also require a certain duration of time between similar services. Give them a call to ensure coverage before your appointment.
- Please note that we do not accept Medicare, Medicaid or DentaQuest.

Cancellations
Please inform us of rescheduling or cancellation requests at least one full business day ahead of time to avoid a cancellation charge. Review our office policies documents above for more information.
